Joining hands to improve grassroots-level healthcare quality

BAC GIANG- Implementing the Project “Rotating qualified health staff from upper-level hospitals to support the lower-levels to improve quality of medical treatment” (known as Project 1816) of Vietnam Ministry of Health, the provincial-level hospitals have sent dozens of health workers and doctors to the grass-roots level hospitals. Thus, the quality of medical examination and treatment at grassroots level has been improve when local people benefit better health service at the localities.

Implementing many collaborative programmes

About 100 patients in Yen Dung district suffering from end-stage renal disease have to maintain hemodialysis. These patients have to go to Bac Giang provincial General Hospital and central-level hospitals three times a week for treatment.

The doctors at the Medical Center of Son Dong district perform eye lens replacement with Phaco surgery.

In order to help patients reduce travel costs, the people’s committee of Yen Dung district allocated 6.5 billion VND (255,392 USD) to support the district Medical Center to equip 10 hemodialysis machines and one RO water purification system in 2023.

To run the equipment, the Center signed a collaborative programme with the provincial General Hospital on dispatching medical staff and doctors to support and instruct implementation of blood filtration process for patients with renal impairment. In line with doctor training, the provincial General Hospital dispatched Dr. Nghiem Tam Duong, Head of the Department of Nephrology- Hemodialysis to the district Medical Center for technical guidance and support during the installation and piloting process.

In the first month running this technique, Dr. Duong always presented to monitor blood filtration cases. As a result, the Center is able to master the technology and maintain hemodialysis for 15 patients after 3 months without any side effects.

In implementation of the Project 1816, in the recent years the provincial-level hospitals have launched a number of programmes and projects to support grassroots-level hospitals. Particularly, Bac Giang Lung Hospital annually dispatches medical staff and doctors to 10 districts, towns and city to train, guide, directly examine, detect and cure tuberculosis patients at the grassroots level. In 2023, the Hospital provided medical examination to more than 51,000 people and detected nearly 1,700 cases with tuberculosis.

Similarly, Bac Giang Obstetrics and Pediatrics Hospital sends dozens of doctors each year to district and communal-level medical centers and private hospitals to support medical care and implementation of new complicated techniques.

Proper selection of contents for support

According to the provincial Department of Health, Project 1816 has contributed to improving technical qualification and skills of lower-level medical practitioners as well as leveling up position and prestige of medical facilities in the localities.

The doctors of the provincial General Hospital support to transfer hemodialysis technology to the Medical Center of Yen Dung district.

Many new and intensive techniques first implemented at district level benefit local people with convenient medical care. Thus, the quality of health examination and treatment at lower-level hospitals has been improved when citizens do not have to go to high-level hospital for treatment helping to reduce costs for patients and ease burdens for higher-level hospitals.

However, medical examination and treatment at district-level medical facilities face numerous difficulties and challenges due to shortage of qualified human resources, thus there is big demand for collaboration in training and support in technology transfer. In order to surmount limitations, the provincial-level hospitals signed collaborative programmes with the district-level medical centers on training and technical transfer since early 2024.

“In order to improve efficiency, we made plans to rotate medical staff to lower-level hospitals based on the targets and requirements of the localities. After the technology transfer, we strengthen supervision and provide continuous training to preserve effectiveness of the new and intensive techniques. For the medical staff dispatched to the grassroots-level hospitals, we request them to make regular report to the hospital leaders on their work performance and keep contact with the localities to timely grasp and exchange information when necessary”, said Hoang Chi Thanh, Vice Director of the provincial General Hospital.
